Now all those guys, whose brains I had managed to brainwash in the last 1 year...And had managed to stop them from believing that I was still working in Coventry......And had finally got them to believe that I now worked in shrewsbury...Here's a surprise. I now work in Telford!! So unclog those clogged up gears in the brain...And think...correlate..(im using pavlov's principle here..kinda)..Every time u see me...Think telford!!...telford!!..TELFORD!! Now im sure by the time u guys register this fact ill be miles away from this place to some other godforsaken place..but at least i tried.
What follows is a part by part account of my life in this place- Princess Royal Hospital. Its a funny thing in this part of the world...in U.K I mean....most of the hospitals r "royal" this or that. when u actually get around to seeing this "royal" hospital..it turns out to be this piddly little, cottage they call a hospital...where a human sitting is more rare than finding a yeti in Sahara. Well, no ..Not that bad...but i just love to exaggerate.
Im on call for the night..that means..The threads of life of over 50 patients lie directly in my hands.....Him ha ha haHAHA!! (evil laughter!!). And..and ...and..hold ur breath..after 12 midnight im also the Surgical doctor on call..so..more patients lives hang by a thread. (more evil laughter!!). No, seriously..its not brain surgery..after a while u get the hang of the job.
Today a child was brought in with a fracture of his femur. Looks quite nasty..considering..femur fracture fragments displace quite a lot owing to force of fracturing force, pull of muscles surrounding the bones and gravity itself. The key to immediate management of such patients..provided their distal neuro vascular status is intact, is to inject a femoral nerve block and manipulate and immobilise the leg ina thomas splint and skin traction, all the while applying traction on the leg. How's that?..femur fracture management for dummies! Further management depends on the degree of angulation and opposition/ overlapping of the two fracture fragments. In veryyoung kids..a greater degree of malalignment is tolerated as the bone remodels quite well say the same for adults. Their femur mostly needs fixing with a intramedullary nail/ enders nail. There u go...so if someone around u has a displaced fracture of femur..u shouldn't get stuck..u can do it too!!
well, that's enough for now. Have to go and put a IV cannula in a confused patient, who has pulled it out for the umpteenth time. I think he does it just to get my attention and get me talking to him. To top it he is a 80 year old guy wit afracture neck of femur!! yuckck!! Well cheerio!!
